CASA volunteers in Arizona are required to keep a monthly contact log/journal to document all activities and communication related to a case. The detailed contact log templates below will help you track things like visits, phone calls, emails, research, and mileage and will provide you with a useful record to refer back to when writing a court report. The month end contact log template is a summary of contacts which you will submit to your CASA Coordinator.
The CASA court report is the vehicle through which CASA volunteers present the information they have gathered about a child's situation and their recommendations about what services will meet the child's needs. Arizona CASA volunteers must submit a draft of the court report at least two weeks prior to the next scheduled court hearing.
